bring them with you to your appointment.
Please always bring a copy of your insurance cards
and remember to bring a list of any medications you are taking. .
Make sure that you have contacted your primary care physician for a referral.
Contact your insurance provider to confirm that vision care is covered. Always bring a photo ID with you.
